BASIC NEEDS DELIVERY REQUEST FORM

-
Vulnerable to COVID-19
-
Elderly
-
Without childcare
-
Without transportation
Deliveries are made on Mondays and Wednesdays only. The cut-off for delivery is noon on the day prior to delivery (e.g. Sunday or Tuesday) to allow 24 hours to pack and organize delivery. Deliveries will most likely be made between 4-5 PM. We will contact the customer to confirm day and time before a delivery is made.
Urgent needs will be processed on a case-by-case basis.
We ask that only people who are unable to leave their homes due to any of the reasons listed below submit a request for delivery. If you are able to leave your home or have already received a delivery in the past month, but still need help, please call us at (570) 374-0181.
Due to demand, completing this form is not a guarantee of delivery and/or specifically requested item(s). We will be fulfilling orders as items are available.
